Citrus Heights Post Acute is seeking a dedicated and compassionate Physical Therapist to join our interdisciplinary rehab team.

Responsibilities:


* Evaluate patients and develop individualized treatment plans


* Provide evidence-based physical therapy interventions


* Collaborate with nursing, therapy, and interdisciplinary teams


* Accurately document treatments and patient progress


* Educate patients and families on recovery and mobility goals

Qualifications:


* Current California Physical Therapist license


* Experience in a skilled nursing or post-acute setting preferred (new grads welcome!)


* Strong clinical, communication, and time-management skills


* Team-oriented mindset with a compassionate approach to care
