We are seeking a passionate, creative, and resident-focused Activities Director to lead a dynamic activity program in a skilled nursing facility.

Key Responsibilities


* Plan, develop, organize, implement, evaluate, and direct resident-centered activity programs in compliance with CMS regulations and state requirements.


* Create and maintain a monthly activity calendar that includes therapeutic group activities, resident outings, and individualized in-room programming for bed-bound or isolated residents.


* Collaborate with the interdisciplinary team to develop and implement resident assessments and individualized activity care plans.


* Design activity interventions that reflect resident interests, abilities, functional levels, and psychosocial needs.


* Encourage resident independence and participation in self-directed activities such as hobbies, crafts, reading, and social engagement.


* Provide adaptive materials as needed, including large print, Braille resources, and audio materials.


* Ensure activity documentation and progress notes are timely, accurate, descriptive, and reflective of resident response and outcomes.


* Maintain effective communication with residents, families, staff, volunteers, community partners, and regulatory agencies.


* Stay current on federal and state regulations, survey guidance, and professional standards related to activity services in skilled nursing facilities.


* Participate in survey readiness efforts and assist with plans of correction related to activity-related deficiencies; provide written reports to the Administrator as needed.
