Position: Social Services Assistant
Location: Magnolia Post Acute Care in East County San Diego
Starting Pay: $20 to $24/hr DOE

Shift:


* Sunday thru Thursday 9am to 5:30pm

Responsibilities:


* Function as patient advocate, helping ensure they have access to services, connecting them with community resources


* Completing quarterly assessments for long term patients


* Supporting discharge planning


* Establishing firm boundaries with patients with goal of helping them achieve maximum autonomy and independence


* Previous SSA or SNF experience required
