Orchard View Post Acute is seeking a compassionate and motivated Physical Therapist to join our interdisciplinary care team.

This role is ideal for a Physical Therapist who is passionate about helping patients regain strength, mobility, and independence in a supportive post-acute setting.

Key Responsibilities


* Evaluate patients and develop individualized physical therapy treatment plans


* Provide skilled therapy services focused on improving mobility, function, and quality of life


* Collaborate with nursing, physicians, and other care team members to support patient-centered outcomes


* Educate patients and families on treatment plans, progress, and safe mobility practices


* Maintain timely and accurate clinical documentation in accordance with regulatory standards


* Uphold the highest standards of clinical care and professionalism

Qualifications


* Current Physical Therapist license in good standing


* Strong commitment to patient-centered, rehabilitative care


* Ability to work effectively as part of an interdisciplinary team


* Experience in post-acute or skilled nursing settings preferred, but not required
