Physical Therapist Assistant (PTA)

San Joaquin Nursing and Rehabilitation
Pay Range: $40-$45 per hour (Based on Experience)

San Joaquin Nursing and Rehabilitation is seeking a motivated and compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team.

Position Summary

The Physical Therapist Assistant (PTA) provides direct patient care under the supervision of a licensed Physical Therapist.

The PTA implements treatment plans, documents patient progress, and collaborates with the interdisciplinary team to ensure optimal outcomes for residents.
Essential Duties and Responsibilities


* Provide physical therapy treatments as directed by the supervising Physical Therapist.


* Implement treatment plans established by the Physical Therapist and physician.


* Document daily treatment notes and weekly progress notes in accordance with state practice acts and payer requirements.


* Communicate resident progress, concerns, and updates to the supervising Physical Therapist and interdisciplinary team.


* Assist nursing department with training of Restorative Aides as appropriate.


* Participate in care plan meetings, Patient Care Conferences, and interdisciplinary team meetings as needed.


* Educate residents and families on exercise programs, safety precautions, and maintenance programs.


* Assist in discharge planning and caregiver training.


* Maintain therapy equipment and ensure treatment areas are clean and organized.


* Report equipment issues promptly to ensure safe and effective operation.


* Maintain professional, respectful communication with residents, families, and staff.

Supervisory Responsibilities

This position does not have supervisory responsibilities.
Qualifications
Education and Experience


* Graduate of an accredited Physical Therapist Assistant program.


* Current, active, and unencumbered PTA license in the State of California.


* Experience in skilled nursing or post-acute setting preferred.

Required Skills and Abilities


* Strong understanding of physical therapy principles and rehabilitation techniques.


* Ability to follow treatment plans and adapt interventions based on resident response.


* Excellent documentation and time management skills.


* Effective communication and teamwork abilities.


* Proficiency with electronic medical records; experience with PointClickCare (PCC) preferred.

Certificates, Licenses, Registrations


* Current Physical Therapist Assistant (PTA) license in good standing.


* Must meet continuing education requirements per state regulations.

* Frequent standing and walking; occasional sitting.


* Frequent reaching, bending, lifting, pushing, and pulling.
