Position Summary

The Staff Occupational Therapist evaluates and treats residents, collaborates with families, physicians, and the interdisciplinary team, and maintains accurate documentation in the medical record.

The OT also conducts in-services and training for facility staff on an ongoing basis.
Essential Duties and Responsibilities


* Evaluate residents within 24 hours (or one business day) of physician referral; document clearly if delayed.


* Develop individualized treatment plans and obtain physician approval.


* Provide skilled occupational therapy services per physician orders.


* Supervise Certified Occupational Therapy Assistants (COTAs), therapy aides, and students in accordance with state practice acts.


* Communicate resident progress, concerns, and care plans with the interdisciplinary team.


* Participate in Care Conferences, Utilization Review meetings, and Rehabilitation Conferences.


* Document evaluations, daily treatment notes, progress notes, and recertifications in compliance with state regulations and payer requirements.


* Instruct families and nursing staff on maintenance programs and caregiver techniques prior to discharge.


* Participate in discharge planning and recommend appropriate durable medical equipment (DME) to enhance resident independence.


* Assist nursing department with training of Restorative Aides.


* Report equipment issues promptly to ensure safe operation.


* Maintain a positive, professional relationship with residents, families, and staff.
