LTC/SNF LPN

Job Title: Licensed Practical Nurse (LPN)
Setting: Skilled Nursing Facility / Long-Term Care (SNF/LTC)
Employment Type: Full-Time, Part-Time, PRN
Shift: Day Shift Only

Position Summary:
We are seeking a dependable and compassionate Licensed Practical Nurse (LPN) to join our SNF/LTC team on day shift.

The LPN will provide direct nursing care to residents while supporting a positive and professional care environment.

Key Responsibilities:


* Provide direct nursing care in accordance with state regulations and facility policies


* Administer medications and treatments as ordered


* Monitor resident conditions and promptly report changes


* Accurately document care provided


* Collaborate with RNs, CNAs, and interdisciplinary team members


* Maintain safety, infection control, and quality care standards

Qualifications:


* Active LPN license in the state of employment


* SNF/LTC experience preferred (new graduates welcome)


* Strong communication and time-management skills


* Compassionate, resident-centered approach
