Rehabilitation Liaison, Brooks Healthcare Systems

The Rehabilitation Liaison is responsible for developing and maintaining referral relationships within Brooks Rehabilitation Hospital’s primary service area.

This position drives patient referrals through direct outreach, education, and collaboration with physicians, acute care hospitals, skilled nursing facilities (SNFs), and assisted living facilities (ALFs).

The Rehabilitation Liaison serves as the face of Brooks Rehabilitation in the community, educating healthcare professionals on our continuum of care, specialized programs, and exceptional outcomes.

Responsibilities


* Develops and maintains strong relationships with physicians, case managers, discharge planners, and healthcare professionals across hospitals, SNFs, ALFs, and physician practices to generate appropriate patient referrals.


* Plans and conducts educational presentations, lunch-and-learns, and in-services to referral sources and perspective patients, highlighting Brooks Rehabilitation’s specialized programs, clinical expertise, and outcomes.


* Attends local and  regional healthcare conferences, networking events, and community health fairs to represent Brooks Rehabilitation and expand professional partnerships.


* Identifies opportunities for collaboration with healthcare providers to increase awareness of Brooks inpatient rehabilitation and post-acute care services.


* Conducts regular outreach visits across the assigned territory, maintaining consistent visibility within key referral settings.


* Tracks and analyzes referral trends, admissions, and market share data to develop targeted growth strategies.


* Works with the marketing and communications department to develop and distribute educational and promotional materials.


* Provides weekly call logs, referral reports, and territory performance updates.


* Identifies new market opportunities and assists in the development of territory-specific strategies and campaigns.


* Educate community, referral sources and physicians on our system of care


* Conducts in-services and presentations as needed.


* Ensures all activities comply with state and federal regulations, including Stark and Anti-Kickback laws.


* Participates in community-based programs and outreach efforts designed to increase awareness of rehabilitation services and patient outcomes.


* Maintains professional visibility in the market by building long-term relationships grounded in education, trust, and service excellence.

Qualifications


* Bachelor’s degree in Marketing, Communications, Healthcare Administration, Nursing, or a related field preferred.


* Minimum of 2 years of experience in healthcare marketing, physician relations, or clinical liaison work; inpatient rehabilitation or post-acute care experience strongly preferred.
