Licensed Practical Nurse (LPN or LVN)
Full Time and Part Time Available
Pay: $32.80 to $44.14 per hour, depending on experience

We are seeking dedicated LPNs or LVNs to provide direct, compassionate care and help lead a strong care team.

Position Summary
The LPN or LVN provides direct nursing care to residents, supports day to day clinical operations, and works closely with the care team to maintain safe, high quality outcomes.

Key Responsibilities
• Provide direct resident care, treatments, and clinical support under physician orders and facility standards
• Administer medications safely, including accurate narcotic counts and medication record reviews
• Complete timely, thorough documentation and charting for admissions, transfers, incidents, and daily care
• Guide and support nursing assistants, ensuring policies, care plans, and safety practices are followed
• Communicate changes in condition, progress, and concerns to physicians, supervisors, and families
• Participate in discharge planning, restorative programs, and interdisciplinary coordination
• Maintain clean, safe treatment areas and follow infection control and workplace safety procedures

Qualifications
• Graduate of an accredited nursing program or approved LPN or LVN program
• Current, active, unencumbered LPN or LVN license in this state
• Able to read, write, and communicate effectively in English
• Skilled nursing or long term care experience is helpful, but not required

Physical Requirements
This role requires regular movement throughout the shift, the ability to lift and assist residents as needed, and comfort working with elderly or medically complex individuals in a fast paced care environment.
