RN Case Manager - Hospice
General Purpose
The Case Manager - Registered Nurse will coordinate and deliver comprehensive, patient‑centered end‑of‑life care.
The RN Case Manager serves as the primary clinical point of contact for patients and families, ensuring that care is individualized, timely, and aligned with the patient's goals and values.
Essential Duties
Patient Care & Case Management
* Serve as the primary nurse responsible for coordinating all aspects of patient and caregiver care
* Conduct comprehensive nursing assessments to identify physical, emotional, psychosocial, and spiritual needs
* Develop, implement, evaluate, and update individualized plans of care in collaboration with the interdisciplinary group (IDG)
* Provide direct nursing care, symptom management, and clinical interventions in accordance with hospice standards and protocols
* Promote patient and caregiver independence whenever possible
* Provide compassionate education and support to patients and families, ensuring understanding of care plans, medications, and disease progression
* Maintain ongoing communication with physicians, team members, and caregivers to ensure continuity of care
* Respond promptly to changes in patient condition and adjust care plans accordingly
* Participate in on‑call rotation as assigned to ensure 24/7 patient support
Interdisciplinary Group (IDG) Collaboration
* Attend and actively participate in IDG meetings and case conferences
* Collaborate with social workers, chaplains, CNAs, volunteers, and other team members to address patient and family needs
* Identify when additional services or disciplines are needed and initiate referrals appropriately
* Maintain strong, respectful working relationships with all team members
Documentation & Compliance
* Complete accurate, timely, and concise clinical documentation in accordance with organizational policies and regulatory requirements
* Ensure all care delivered meets state, federal, and accreditation standards
* Maintain thorough records of assessments, interventions, communications, and care plan updates
Professional Accountability
* Maintain current RN licensure, CPR certification, and required continuing education
* Demonstrate competency in hospice nursing practices, symptom management, and communication skills
* Stay informed of current trends, best practices, and regulatory changes in hospice and palliative care
* Represent the organization professionally with patients, families, healthcare partners, and community agencies
* Exhibit strong time‑management skills and the ability to prioritize in a dynamic environment
* Perform additional duties as assigned to support patient care and team operations
Qualifications ( Education and/or Experience)
* Current, active, and unrestricted RN license (compact/multistate license preferred)
