South Creek Post Acute is seeking a dedicated Admissions Nurse to join our team.

This position plays a key role in coordinating patient admissions and ensuring a smooth transition into our facility.

Position Details:


* Schedule: Monday-Friday, 11:00 AM - 7:00 PM


* License: RN preferred; LPN will be considered


* Pay Range: $40-$55 per hour, based on credential and experience

Key Responsibilities:


* Coordinate and complete patient admissions


* Review and verify admission documentation


* Communicate with hospitals, families, and interdisciplinary team members


* Ensure compliance with facility policies and regulatory requirements


* Provide a positive admission experience for new residents

Qualifications:


* Active RN or LPN license in good standing


* Strong communication and organizational skills


* Experience in post-acute, skilled nursing, or admissions preferred


* Ability to work independently and collaboratively
