About the Facility: Diablo Valley Post Acute is a leading provider of short-term rehabilitation and long-term care.

We operate a 7-day-a-week therapy program, ensuring our patients receive consistent, high-quality care.

We are seeking a licensed PT who is dedicated to clinical excellence and interdisciplinary collaboration.

Clinical Responsibilities:



* Perform comprehensive evaluations and develop physician-approved treatment plans.


* Direct patient care and supervision of Physical Therapy Assistants (PTAs) in accordance with CA state practice acts.


* Collaborate with the interdisciplinary team to communicate patient progress and goals.


* Provide education to nursing staff, patients, and families regarding maintenance and caregiving.


* Lead discharge planning and recommend necessary durable medical equipment (DME).


* Maintain compliant and timely documentation of all clinical services.

Requirements:
