Position Responsibilities


* Conduct comprehensive nutritional assessments for residents


* Develop, implement, and monitor individualized nutrition care plans


* Collaborate with physicians, nursing staff, speech therapy, and dietary services


* Monitor weight trends, lab values, wounds, and overall nutritional status


* Provide education to residents, families, and staff as needed


* Ensure compliance with state, federal, and facility regulations


* Participate in care plan meetings and interdisciplinary team discussions


* Oversee therapeutic diets and ensure accurate diet implementation

Qualifications


* Registered Dietitian (RD) credential, active and in good standing


* Current state licensure (if applicable)


* Experience in skilled nursing or long-term care preferred


* Strong clinical assessment and documentation skills


* Ability to work independently and as part of a team


* Passion for improving the quality of life for the elderly
