Position Summary: This position is responsible for providing comprehensive support to residents and their families throughout the skilled nursing experience.

In addition to addressing emotional and psychosocial needs, this role oversees all aspects of case management, ensuring residents and families receive the guidance, resources, and advocacy necessary to navigate admissions, transitions of care, discharge planning, and ongoing support services. 

Job Responsibilities:
1.

Plan, develop, organize, implement, evaluate, and direct the social service programs at this facility and ensure standardization of social services across Aging Services division.
2.

Manage social services department staff, including hiring, performance management, and development. 
3.

Assist and participate in developing, implementing and conducting relevant in-service training programs.
