* Evaluate patients and create individualized treatment plans


* Improve strength, mobility, balance, and functional independence


* Deliver skilled therapy in a post-acute setting


* Support safe, successful discharges


* Collaborate with nursing and the interdisciplinary team


* Complete timely and accurate documentation

What You Need


* Active California Physical Therapist License required


* Skilled nursing or post-acute experience preferred but not required


* Strong clinical judgment and communication skills
