Job Summary

The Social Services Assistant supports the Social Service Director/Consultant in planning, organizing, implementing, and maintaining social service programs to meet the emotional, social, and psychosocial needs of residents.

This position works closely with nursing, administration, and the interdisciplinary care team to support resident well-being and successful discharge planning.

Responsibilities


* Assist the Social Service Director/Consultant with daily social services operations


* Support residents and families with adjustment to facility admission and ongoing care


* Provide information on Medicare/Medicaid and other financial assistance programs


* Document social service progress notes and resident responses to care plans


* Coordinate with nursing, medical staff, and other departments to meet resident needs


* Assist with discharge planning, including referrals, resources, and equipment needs


* Help connect residents with community agencies and support services


* Assist with scheduling outpatient appointments and on-site ancillary services (optometry, podiatry, dentistry, psychiatric services, etc.)


* Maintain confidentiality and protect resident information (PHI)


* Report suspected fraud or unauthorized disclosure of resident information

Qualifications (Preferred)


* Experience in a skilled nursing facility (SNF), healthcare, or social services preferred


* Strong communication, organization, and documentation skills


* Ability to work independently and as part of a team


* Compassionate and professional approach with residents and families

Pay

💵 $18-$25/hour (based on experience)
