Licensed Vocational Nurse (LVN) - Full-Time AM Shift

Location: Sunnyvale Post Acute
Schedule: 4 on / 2 off rotation
Pay Rate: $40-$45 per hour (DOE)
Status: Full-Time

We are looking for a reliable, experienced LVN to join our team on a full-time AM shift.

Position Summary

The LVN plays an essential role in supporting our residents' health and wellbeing by providing skilled nursing care under the direction of the DON and RN leadership team.

This position requires strong clinical judgement, a positive attitude, and the ability to thrive in a fast‑paced SNF environment.

Schedule

✔ AM Shift
✔ 4 days on / 2 days off repeating schedule
✔ Consistent full-time hours

Responsibilities


* Provide direct nursing care to residents in accordance with care plans and regulatory guidelines


* Administer medications and treatments accurately and timely


* Monitor residents for changes in condition and report to RN/DON


* Document care, interventions, and observations in EMR


* Communicate effectively with residents, families, and interdisciplinary team


* Uphold infection control, safety standards, and facility policies


* Support a caring, respectful, team-oriented environment

Requirements


* Active California LVN license


* Experience working in a Skilled Nursing Facility (SNF) required


* Strong clinical assessment and communication skills


* Ability to work independently and as part of a collaborative care team

Compensation

$40-$45 per hour, depending on experience.
