Your Responsibilities


* Evaluate and develop individualized treatment plans to improve resident function and mobility


* Collaborate with nursing, therapy, and medical teams to achieve optimal outcomes


* Document progress and adjust treatment as needed to support rehabilitation goals


* Provide education and support to residents and families

Qualifications


* Current California Physical Therapist (PT) license


* Skilled Nursing or post-acute experience preferred, but not required


* Compassionate, reliable, and committed to clinical excellence
