Pay: $57-$67 per hour (DOE) + Full Benefits

Sunnyvale Gardens Post Acute is seeking a skilled and patient‑centered Physical Therapist (PT) to provide high‑quality rehabilitation services to our residents.

This is a full‑time on‑site role offering a competitive hourly rate, comprehensive benefits, and the opportunity to make a meaningful impact every day.

What You'll Do


* Evaluate residents and develop individualized treatment plans based on clinical findings.


* Provide high‑quality physical therapy interventions to support mobility, strength, balance, and functional independence.


* Document evaluations, progress notes, and care plans in accordance with state and facility guidelines.


* Collaborate with an interdisciplinary team including nursing, occupational therapy, speech therapy, and social services.


* Educate residents, families, and caregivers on therapy programs and safe mobility strategies.


* Participate in care conferences and contribute to discharge planning.

What We're Looking For


* Valid California Physical Therapist (PT) license in good standing.


* Graduate of an accredited Physical Therapy program.


* Strong clinical judgment and a passion for post‑acute and geriatric care.


* Excellent communication skills and ability to work collaboratively in a team‑oriented environment.


* Experience in a skilled nursing facility (SNF) preferred, but new grads are welcome to apply!
