Role Overview
Shadelands Post Acute is seeking a detail-oriented and clinically sound Licensed Vocational Nurse (LVN) to step into the vital role of MDS Coordinator.
In this position, you are the architect of resident assessments and the guardian of clinical reimbursement.
You will ensure that every resident's care is accurately reflected in the Minimum Data Set (MDS) in accordance with federal and state regulations, directly impacting the quality of care and the facility's operational success.

Key Responsibilities
* MDS Management: Coordinate and complete MDS assessments and Care Area Assessments (CAAs) within strict regulatory timelines.
* Clinical Accuracy: Review clinical documentation to ensure it accurately supports resident care needs, care planning, and appropriate reimbursement.
* Compliance Leadership: Maintain full compliance with CMS, state, and facility guidelines, staying current with all regulatory changes.
* Interdisciplinary Collaboration: Work closely with therapy, social services, dietary, and nursing teams to build comprehensive, individualized resident care plans.
* Audit Readiness: Assist with audits and state surveys by providing precise documentation and detailed reports.
* Staff Education: Provide guidance to the nursing team on documentation standards and MDS requirements to ensure facility-wide excellence.
* Quality Improvement: Participate in resident care conferences and Quality Assurance (QAPI) initiatives to promote superior resident outcomes.

Qualifications
* A current, valid California LVN License.
* MDS Experience Preferred: Knowledge of MDS 3.0, the RAI process, and Medicare/Medicaid reimbursement guidelines.
* Skilled Nursing Background: Prior experience in a long-term care or skilled nursing facility (SNF) is highly preferred.
