About the Role

Concord Post Acute is seeking a dedicated and compassionate PRN Physical Therapist Assistant (PTA) to join our rehabilitation team.

In this role, you will provide vital support under the direct supervision of a licensed Physical Therapist, assisting with patient-related activities to help our residents regain mobility, manage pain, and achieve their rehabilitation goals.

Essential Duties & Responsibilities


* Administer physical therapy treatments and procedures as directed by the Physical Therapist.


* Document daily treatment notes, weekly progress notes, and other required records accurately and timely, in accordance with state regulations.


* Assist with the maintenance and cleaning of the therapy department and equipment.


* Participate in patient care and rehabilitation conferences as needed.


* Train restorative aides in the nursing department as instructed.


* Communicate effectively with the Physical Therapist, supervisor, and other healthcare team members regarding patient progress, concerns, and treatment plans.


* Educate patients, their families, and nursing staff on at-home maintenance programs and caregiver training in preparation for discharge.


* Report any issues with departmental equipment to ensure it remains in good working order.


* Maintain a positive, effective, and professional rapport with patients, residents, families, and colleagues.

Qualifications


* Licensure: Active and current Physical Therapist Assistant (PTA) license in the state of California.
