Canyon Springs Post Acute is seeking a skilled and motivated Physical Therapist (PT) to join our premier therapy team on a PRN basis.

Key Responsibilities



* Clinical Leadership: Evaluate residents and design evidence-based treatment plans, ensuring all services are approved by referring physicians.


* Direct Treatment: Deliver high-quality, hands-on therapy in accordance with the established plan of care and the California state practice act.


* Supervision & Mentorship: Oversee Physical Therapist Assistants (PTAs) and provide guidance to Restorative Aides to ensure consistent care.


* Precise Documentation: Maintain accurate evaluations, daily notes, progress reports, and recertification documentation in a timely manner.


* Interdisciplinary Collaboration: Work closely with nursing and the broader clinical team; participate in care conferences and utilization reviews as needed.


* Discharge Readiness: Educate residents, families, and staff on safe mobility techniques and home-exercise programs to support successful transitions.


* Equipment Advocacy: Recommend and coordinate necessary durable medical equipment (DME) such as walkers, braces, or grab bars.

What You Bring



* Current, unrestricted Physical Therapist (PT) License in the state of California.


* Strong clinical reasoning and accurate documentation skills.


* The ability to motivate residents and build positive relationships with families and colleagues.
