Compensation: $ 57.00-$ 67.00 per hour (Commensurate with experience)

Key Responsibilities



* Clinical Excellence: Develop and implement evidence-based treatment plans tailored to short-term rehab and long-term care residents.


* Rehab Leadership: Supervise Physical Therapy Assistants (PTAs) and collaborate with the nursing department to train Restorative Aides.


* Interdisciplinary Teamwork: Actively participate in Patient Care Conferences, Utilization Review, and Rehabilitation meetings to ensure a holistic approach to recovery.


* Precision Documentation: Complete evaluations, daily notes, and weekly progress reports in accordance with California PT Board standards and facility policies.


* Discharge Mastery: Lead the discharge planning process, including family education, caregiver training, and recommending necessary durable medical equipment (DME).


* Advocacy: Communicate effectively with physicians and supervisors regarding resident progress, ensuring all treatment plans are optimized for success.

Qualifications



* Current, valid Physical Therapist (PT) License in the state of California.
