Compensation: Starting Salary: $75,000 per year

General Purpose

Delta View Post Acute is seeking a proactive and empathetic Social Services Director to lead our resident advocacy and psychosocial support programs.

The ideal candidate is an organized, objective professional who excels at building meaningful relationships with residents and their families while maintaining the highest standards of confidentiality and clinical excellence.

Essential Duties



* Resident Advocacy: Establish courses of action by exploring options and setting goals with residents and their families to achieve the highest level of independence and well-being.


* Psychosocial Care: Provide medically related social services to ensure the highest practicable physical, mental, and psychosocial well-being for every resident.


* Assessment & Care Planning: Evaluate psychological and emotional needs to develop comprehensive social service care plans.


* Emotional Support: Offer specialized counseling for residents and families coping with physical disabilities, health-related fears, and grief associated with loss or end-of-life.


* Resource Coordination: Assist residents with healthcare decisions, securing prosthetics/assistive devices, and obtaining community resources from social service and welfare agencies.


* Resident Services: Manage personal shopping assistance, inventory and track patient belongings, and coordinate responses to reports of lost or missing items.


* Ancillary & Discharge Coordination: Schedule on-site ancillary services (optometry, podiatry, dentistry, psychiatry) and lead the discharge planning process in coordination with the interdisciplinary team and outside agencies.


* Departmental Collaboration: Maintain strong working relationships with all facility departments and community partners to ensure seamless service delivery.


* Education: Provide in-service training to facility staff regarding social service protocols and resident rights.

Qualifications



* Experience: Minimum of 2 years of supervised social work experience in a healthcare setting, working directly with individuals in a clinical or post-acute environment.


* Skills: Exceptional listening and verbal communication skills; ability to remain objective and organized under pressure; persistence in navigating complex social and regulatory systems.

Supervisory Requirements



* This position is an individual contributor role and has no direct supervisory responsibilities.
