Afterhours Hospice Visit RN (24 hours)

Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties.

With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

Title: Hospice Visit RN - Afterhours
Schedule: 5pm to 5am on Wednesday & Thursday
Location: Field-based, varies throughout the territory based on patient need.

Mileage reimbursement is provided at 72.5 cents per mile
Pay: $42.34 to $50.81 per hour

ESSENTIAL DUTIES AND RESPONSIBILITIES


* The Afterhours Registered Nurse will primarily conduct afterhours home visits and provide care to patients, as delegated by the Phone Triage Nurse.


* Provides nursing care to meet the patient’s needs in accordance with the patient’s hospice plan of care.


* Cross-train on phone triage to assist Triage RN team as necessary.


* Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data.

Establishes measurable patient goals and communicates an on-going plan of care appropriately to the interdisciplinary team (IDT).

Recognizes changes in the clinical status of the patient and reports findings to the appropriate team member.


* Collaborates with patient/family when developing the plan of care, adjusting communication style as necessary.

EDUCATION & REQUIREMENTS


* Must possess a current RN license in the state of Colorado


* Must have a minimum of one year RN experience – hospice or home health experience highly preferred


* Must successfully pass required background checks and drug screening


* Requires travel to multiple work locations
