Community Health Worker

The Community Health Worker (CHW) serves as an advocate or liaison in the community setting.

The position works in partnership with the community and plays a critical role in connecting networks with the community.

The CHW will play a central role in recruiting families, providers, and organizations throughout the Needs Assessment phase.

They will also lead stakeholder engagement for the Community Advisory Board, identifying caregivers, youth with asthma, pediatric healthcare providers, school health staff, and other community partners who can participate.

Essential Functions:

Excellent verbal and written communication skills, and the ability to work effectively with a wide range of constituencies in a diverse community and to build trust.

Identifying and developing relationships with key community partners

Community engagement, outreach and/or events planning

Locate and research new services and resources

Recruitment of families for needs assessments

Maintain records in accordance with program and funding source requirements

Develops a weekly calendar of outreach activities and location

Conducts formal presentations in the community
Participate in trainings, meetings, workshops, conferences, webinars, and conference calls as necessary

Qualifications:


* High school diploma required


* At least one year of relevant experience required


* Strong problem solving and analytical skills Knowledge of Microsoft Office (Word, Excel, Outlook, and PowerPoint)


* Knowledge of virtual platforms
