Visalia Post Acute is seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team.

This is an excellent opportunity for a motivated COTA who is passionate about helping residents achieve their highest level of independence and quality of life in a skilled nursing and rehabilitation setting.
General Purpose

The Certified Occupational Therapy Assistant (COTA) assists the Occupational Therapist in providing patient-centered occupational therapy services.

This role includes delivering direct patient care, implementing treatment plans, documenting progress, and collaborating with the interdisciplinary care team in accordance with physician orders, state practice acts, and facility policies.
Essential Duties and Responsibilities


* Provide treatment to residents as directed by the supervising Occupational Therapist


* Follow physician-approved treatment plans and modify activities as appropriate within scope of practice


* Record daily treatment notes, weekly progress notes, and recertification documentation per OT Board, facility, and payer requirements


* Communicate resident progress, challenges, and care plans with the Occupational Therapist and interdisciplinary team


* Educate residents, families, caregivers, and nursing staff on home programs, safety techniques, and discharge planning


* Participate in discharge planning and coordination of care


* Assist nursing department with training of Restorative Aides as needed


* Participate in in-service education and facility training programs


* Maintain clean, safe, and organized treatment areas


* Report equipment issues promptly to ensure safe and effective operation


* Maintain professional, respectful relationships with residents, families, and staff

Supervisory Responsibilities


* This position has no supervisory responsibilities

Qualifications

Education & Experience


* Graduate of an accredited Occupational Therapy Assistant program


* Skilled nursing or geriatric experience preferred but not required

Licensure & Certifications


* Current, active California Certified Occupational Therapy Assistant (COTA) license in good standing


* Must meet all continuing education requirements per state regulations

Skills


* Proficient in electronic documentation systems (Casamba and PointClickCare preferred)


* Strong communication, organizational, and clinical skills


* Ability to work independently and collaboratively within a team environment
