What You'll Do


* Evaluate patients + create personalized treatment plans


* Improve mobility, strength, balance, and functional independence


* Support safe discharges through progress-based care


* Document accurately and collaborate with the care team

Requirements


* Active California PT License (required)


* Skilled nursing/post-acute experience is a plus (not required!)


* Team-first attitude + strong patient care
