What You'll Do


* Provide individualized physical therapy services to residents


* Develop treatment plans to improve mobility, strength, and independence


* Collaborate with interdisciplinary team members to ensure holistic care


* Document resident progress in compliance with state and federal guidelines


* Educate residents and families on safe mobility and home programs

Qualifications


* Current California Physical Therapist (PT) license required


* Strong clinical skills and passion for patient-centered care


* Excellent communication and teamwork abilities


* SNF or post-acute experience preferred, but not required (mentorship available)
