Vocational Case Manager Job Responsibilities:


* Completes Intake Assessments for all residents of DePalmer House.


* Participates in the development of an individualized service plan for assigned clients; coordinates and facilitates the delivery of services in accordance with service plan.


* Understands and applies case management principles to increase DePalmer House resident self-sufficiency and transition from unemployment to competitive employment.


* Responds effectively to client emergencies, which may require flexibility of both time and areas of assignment.


* Establishes relationships with and utilizes community service providers and employers for a broad range of client needs, including referral to housing, medical care, addiction treatment, mental health counseling, educational and vocational services.


* Completes all required documentation, in timelines in accordance with program standards.

Vocational Case Manager Qualifications:

Bachelors/Associates Degree and two (2) years’ related experience in vocational services.

Must possess a valid New York State Driver’s License and have access to reliable transportation.

Vocational Case Manager Required Skills and Abilities:

Strong organizational and interpersonal skills.

Ability to develop and promote employment programs with area employers and service providers.

Ability to work successfully within outcome-driven program.

Demonstrates initiative and sound judgment.

Strong written and verbal communication skills necessary.

Pay: $20.19 per hour
