Job Description:


* Treat patients as directed by the Occupational Therapist.


* Record daily treatment notes and weekly progress notes per Occupational Therapy Board requirements.


* Implement treatment plans per the physician's orders.


* Assist the nursing department with training of Restorative Aides.


* Communicate with supervisors and other health team members regarding patient progress, problems, and plans.


* Participate in in-service training programs for other staff in the facility.


* Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per the Occupational Therapy Board State Practice Act and governmental and third-party payer requirements.


* Document treatment changes according to policies and procedures.


* Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services.


* Participate in discharge planning efforts.


* Assist with the cleaning and maintenance of the treatment area.


* Report any issues with department equipment to ensure it is maintained in good working order.


* Establish positive, effective relationships with patients/residents, families, staff, and professional colleagues.

Required License or Certification


* Valid Occupational Therapist Assistant License
