We are a rehab-focused team that serves both short-term and long-term patients.

We are currently seeking a passionate and motivated Occupational Therapist to join our dynamic in-house therapy team.

Key Responsibilities


* Develop and implement effective treatment plans in coordination with referring physicians.


* Provide direct therapy services following the approved plan of care.


* Train and support Restorative Aides within the nursing department.


* Supervise Occupational Therapy Assistants (OTAs) in patient care and documentation per state practice guidelines.


* Collaborate with supervisors and interdisciplinary team members to monitor progress and adjust treatment as needed.


* Participate in care conferences, utilization reviews, and rehabilitation meetings as required.


* Maintain accurate and timely documentation, including evaluations, daily notes, progress reports, and recertifications per regulatory standards.


* Educate patients, families, and caregivers on home programs and discharge planning to promote continued independence.


* Recommend and coordinate the use of durable medical equipment (DME) to support safe mobility and function (e.g., braces, walkers, grab bars).


* Report equipment issues promptly to ensure a safe and functional work environment.

Qualifications


* Licensure: Valid Occupational Therapist License (State of California)
