Position Summary

As a CNA at Lincoln Meadows Care Center, you will provide essential hands-on care and support to residents while maintaining dignity, comfort, and safety.

You will work closely with the nursing team to deliver high-quality, resident-centered care in accordance with individualized care plans.

Key Responsibilities (Summary):


* Assist residents with activities of daily living, including bathing, dressing, grooming, feeding, and mobility


* Monitor and report changes in resident condition to the nursing team


* Accurately document care, vital signs, and intake/output as directed


* Respond promptly to resident needs and call lights


* Support admissions, transfers, and discharges to ensure resident comfort


* Maintain a clean, safe, and calm environment


* Observe and report skin conditions, pressure areas, and safety concerns


* Follow infection control, safety, and confidentiality guidelines


* Work collaboratively with nursing and interdisciplinary teams

Qualifications:


* Current Certified Nursing Assistant (CNA) license in good standing in the State of California


* CPR certification preferred


* Compassionate, dependable, and team-oriented


* Strong communication and observational skills


* Commitment to providing respectful, resident-focused care
