Pay: $75,000-$100,000/year DOE
Schedule: Full-Time

Key Responsibilities


* Lead and manage the Social Services department, ensuring residents receive emotional support and discharge planning services


* Collaborate with residents, families, and interdisciplinary teams to develop individualized care plans


* Build strong community partnerships and advocate for residents' rights


* Promote a resident-centered environment through empathy, communication, and problem-solving

Qualifications


* Strong communication, organization, and leadership skills


* Dependable, caring, and committed to serving the elderly


* Prior experience in healthcare or skilled nursing social services (preferred)


* Degree in Social Work or related field (required)
