Midtown Oaks Post Acute is hiring a Physical Therapist (PT) to join our dynamic rehab team in a skilled nursing/post-acute care setting.

We are looking for a compassionate, motivated clinician who values quality care and teamwork.

Pay Range:


* $60-$70/hour, based on experience

Physical Therapist (PT) Responsibilities:


* Evaluate and treat patients in a skilled nursing/post-acute setting


* Develop and implement individualized treatment plans


* Document evaluations, progress notes, and discharge summaries accurately and timely


* Collaborate with nursing, therapy, and interdisciplinary teams


* Educate patients and families on therapy goals and home exercise programs

Physical Therapist (PT) Requirements:


* Active California Physical Therapist license (or pending)


* Skilled nursing or post-acute experience preferred, but not required


* Strong communication and time-management skills


* Team-oriented, compassionate, and patient-focused
