Position Overview:

Sunnyvale Post Acute is seeking a Rehab Aide to assist in providing treatment and support services for rehabilitation under the direct supervision of a licensed therapist.

This role contributes to the overall positive image of the organization and ensures that all required documentation and forms are appropriately completed and included in patient charts.

Job Description:


* Assist with and perform setup as assigned for physical and occupational therapy treatments; demonstrate knowledge of treatment goals and precautions while assisting or setting up for patients.


* Ensure the safety and cleanliness of the work area and equipment.


* Maintain open communication with facility staff.


* Confer with therapists to establish treatment schedules.


* Transport, transfer, and facilitate patient care as directed by the therapist.


* Prepare patient equipment and supplies for treatment.
