Responsibilities:

As the Social Services Director, you will:


* Lead the planning and implementation of resident-focused social services programs.


* Offer emotional support and counseling to residents and their families.


* Manage and coordinate the discharge planning process in collaboration with the care team and external agencies.


* Schedule and arrange all necessary on-site and off-site resident services.


* Oversee residents' practical needs, including personal shopping and management of resident belongings.


* Maintain effective and empathetic communication with all stakeholders involved in resident care.

Qualifications:


* Bachelor's degree in Social Work or a related field (MSW preferred).


* 2+ years of supervised social work experience in a healthcare setting.


* Ability to handle complex and confidential situations with discretion.


* Strong organizational and communication skills.


* Proactive and persistent approach to problem-solving.
