Responsibilities:


* Provide high-quality nursing care to residents under the direction of the RN and DON


* Administer medications and treatments accurately and timely


* Monitor and record resident health status and communicate changes promptly


* Foster positive relationships with residents, families, and interdisciplinary team members


* Ensure compliance with all facility policies and state/federal regulations

Requirements:


* Valid Ohio LPN license in good standing


* Previous long-term care or rehabilitation experience preferred


* Strong communication and teamwork skills
