MDS Coordinator - Marin Post Acute
Location: San Rafael, CA
Pay: $90,000-$130,000 annually (DOE)
Schedule: Full-Time

Marin Post Acute is seeking a skilled and detail-oriented MDS Coordinator (LVN or RN) to join our leadership team.

What You'll Do


* Lead and manage the full MDS/RAI process in accordance with federal and state guidelines


* Accurately complete assessments, care plans, and documentation


* Collaborate with IDT members to ensure individualized, resident-centered care


* Monitor compliance, quality measures, and reimbursement accuracy


* Provide education and support to staff regarding MDS processes and updates


* Work closely with administration and clinical leadership on audits, reporting, and workflow improvement

What We're Looking For


* RN or LVN with current California license


* MDS/RAI experience required (minimum 1 year preferred)


* Strong understanding of PDPM, care planning, and regulatory compliance


* Excellent attention to detail and organizational skills


* Strong communication and teamwork abilities
