The Community Health Worker (CHW) supports patients at Salinas Valley Health by addressing social determinants of health, assisting with Medi-Cal enrollment and renewal processes, and helping individuals with complex medical, behavioral, and social needs successfully navigate the healthcare system.

This role focuses on reducing avoidable hospitalizations and Emergency Department utilization by connecting patients with essential resources, coaching them toward self-management, and supporting continuity of care after discharge.

Primary Duties:


* Assist uninsured or underinsured patients with Medi-Cal enrollment, Hospital Presumptive Eligibility (HPE) applications, and Medi-Cal redetermination/renewal.


* Conduct social determinant of health assessment, identify barriers and connect patient with applicable community based resources.


* Provide individualized goal-setting support and promote self-management.


* Maintain regular contact with assigned patients to reduce ED visits and support treatment adherence.


* Collaborate with case management, nursing, pharmacy, social work, and other partners.


* Participate in rounds, huddles, and case review meetings.


* Document all interactions accurately in the EHR.


* Serve as a resource for staff and community partners.


* Support program development and quality-improvement initiatives.


* Performs other duties as assigned.

Job Requirements:

Education: High school diploma or GED required.

Bachelor's degree preferred.

Licensure: Valid Driver's License required.

Community Health Worker Certification required.

New hires/transfers have eighteen (18) months to obtain.

Ability to read and speak Spanish fluently required.

Experience: Experience in healthcare, social services or enrollment services preferred.

Essential Technical/Motor Skills: Demonstrates sufficient manual dexterity to operate office equipment, including computer terminals, printers, copiers with reasonable speed and ability to move throughout the Hospital.

Interpersonal Skills: Work effectively with multidisciplinary teams, including physicians and other staff.

Remain calm in high-stress situations.

Adapt to changing patient volume and demands.

Communicate clearly and professionally.

Essential Mental Abilities: Excellent clinical judgment and decision-making skills.

Ability to organize, prioritize and apply critical thinking process.

High level of mental concentration and multiple tasks performed simultaneously with frequent interruptions in stressful conditions.

Maintains strict confidentiality of patient data.

Pay Range: The hourly rate for this position is $33.19 - $40.00.
