Linwood Meadows Care Center, a premier skilled nursing facility in Visalia, CA, is hiring a Per-Diem Physical Therapist Assistant (PTA) to join our dedicated rehabilitation team.

Position Summary:
The Physical Therapist Assistant provides physical therapy treatments under the direction and supervision of a licensed Physical Therapist.

This role supports residents in achieving their mobility and recovery goals as part of an interdisciplinary care team.

Key Responsibilities:


* Deliver therapy services in accordance with the treatment plan established by the Physical Therapist


* Monitor patient progress and communicate findings to the supervising therapist


* Accurately document daily treatments, patient responses, and progress using systems such as Casamba and PointClickCare


* Assist with discharge planning and family education regarding home exercises or mobility assistance


* Ensure compliance with all federal, state, and facility guidelines and policies


* Collaborate with nursing, therapy, and administrative teams to support quality resident care

Qualifications:


* Associate's degree from an accredited PTA program


* Current PTA license in good standing in the State of California


* Skilled nursing or post-acute care experience preferred


* Strong communication, teamwork, and organizational skills


* Proficient with electronic medical records and therapy documentation platforms
