Kern River Transitional Care Center is seeking a dedicated and compassionate Physical Therapist to join our rehab team on a Per Diem basis.

Responsibilities:


* Evaluate and assess patients to develop personalized treatment plans


* Provide high-quality, hands-on therapy in accordance with physician orders


* Accurately document treatments and patient progress


* Communicate effectively with the interdisciplinary team


* Educate patients and caregivers on at-home therapy and safety strategies

Qualifications:


* Active California PT license in good standing


* CPR certification


* Experience in skilled nursing or rehab preferred


* Excellent interpersonal and documentation skills
