Telehealth Physical Therapist (PT) - Sonoma Post Acute

Pay: From $75 per hour
Schedule: Flexible | Remote | Part-time or PRN

Sonoma Post Acute is seeking a Telehealth Physical Therapist to provide high-quality virtual care to our residents.

This role is ideal for a licensed PT who values flexibility, strong teamwork, and delivering effective patient outcomes through telehealth.

Responsibilities


* Conduct virtual PT evaluations and follow-up treatments


* Develop individualized care plans


* Document accurately in the EMR


* Collaborate with onsite nursing and rehab teams


* Support continuity of care for post-acute residents

Qualifications


* Active California Physical Therapist license


* Experience in SNF, post-acute rehab, outpatient, or telehealth preferred


* Strong communication and documentation skills


* Comfortable working independently in a remote setting
