With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.
Title:Â PACE Home Care Coordinator
Schedule:Â Monday-Friday from 8am to 5pm
Location:Â TRU PACE in Lafayette, CO
Pay:Â $23.10 to $28.85 per hour
JOB SUMMARYÂ
The PACE Home Care Coordinator is responsible for scheduling participant home care nursing visits as requested from provider, RN Case Manager and Clinic and Nursing Manager.
Coordinates and schedules homecare initial and reassessment appointments to make certain that CMS regulations are followed to ensure that all participants are receiving updated assessments appropriately.

RESPONSIBILITIESÂ
* Responsible for the scheduling of home care nursing initial and reassessment appointments, as well as regulatory home care CNA supervisory visits under the direction of the Clinic and Nursing Manager.Â
* Collaborating up to monthly with contracted agencies to review care plans, services, frequency of visits being scheduled, frequent cancelations, to help ensure that the necessary services are being provided and communicating with Clinic and Nursing Manager on outcome of collaboration meeting.Â
* Meeting regularly (weekly) with home care nurses to review current goals and priorities for the week.Â
* Monitoring and assigning tasks in Greenway to assist with completing orders entered by the PCPâs.Â
* Participates as a member of the interdisciplinary team (IDT).⯠Maintains regular attendance at, and participates in all IDT meetings, communicating participant changes, collaborating on care planning decisions and coordination of 24-hour care delivery.Â
* Effectively identifies, responds to, recommends and/or resolves problems as they arise in collaboration with Manager, and IDT.Â
* Serve as liaison with participants and the home care nurses, including problems solving, appointment planning and schedule changes.Â
EDUCATION & QUALIFICATIONS
* Regular high school or holiday high school degreeÂ
* 1-2 years relevant experience working in scheduling data bases (Preferred).Â
* Experience working with individuals with mental health needs (Preferred).Â
* Experience working with the frail or elderly population (Preferred).Â
* Experience with Electronic Medical Records (EMR).Â
