Resident Care Manager

Wage: $35-$60 per hour (depending on licensure)

RN: $44-$60 (DOE) & LPN: $35-$47 (DOE)

Key Responsibilities

Care Coordination


* Develop, update, and monitor individualized resident care plans.


* Ensure plans reflect the physical, emotional, and psychosocial needs of each resident.


* Complete routine assessments and report changes in condition to the appropriate providers.

Resident Care & Advocacy


* Monitor resident health and provide direct nursing care as needed.


* Promote dignity, safety, and resident rights at all times.


* Serve as the main point of contact for families regarding resident care.

Team Leadership


* Support and supervise CNAs and nursing staff.


* Provide coaching, feedback, and ongoing training.


* Assist with performance evaluations and corrective action processes.

Documentation & Compliance


* Accurately document assessments, interventions, and care in the medical record.


* Follow state, federal, and facility regulations, including infection control and safety protocols.


* Participate in state surveys, audits, and quality assurance programs.

Communication & Collaboration


* Coordinate care with physicians, therapists, and other healthcare professionals.


* Facilitate resident and family care conferences.


* Report changes, incidents, and concerns promptly to the Director of Nursing.

What You Bring


* Current, unencumbered LPN or RN license in Washington State (required).


* 2 years of skilled nursing, long-term care, or similar experience preferred.


* Strong understanding of long-term care regulations.


* Excellent communication, organization, and leadership skills.


* Ability to prioritize, multitask, and adapt to resident needs.
