Full-time Remote Position following training in Indianapolis

Monday - Friday, 8:00 AM - 5:00 PM

Must live within driving distance of Indianapolis

What does a Transitional Care Coordinator do each day at ASC?

The Transitional Care Coordinator supports our business development team by strengthening collaboration across our hospital, ACO, and community partners.

This role plays a key part in improving patient outcomes by maintaining excellence in communication and follow through.


* Work with facility teams to maintain accurate and up-to-date patient data within various population health platforms.


* Provide timely updates on patient status, barriers to discharge, and progress toward goals.


* Generate reports, dashboards and insights for leadership, ACO partners, and clinical teams.


* Identify trends in readmissions and opportunities for improved care pathways.


* Provides timely updates to hospital partners regarding placement availability.


* Manages population health software and provides training as needed.

Requirements


* Previous healthcare experience preferred.


* Familiarity with population health software (e.g.

Wellsky, EPIC, Watershed, Olio, etc.) preferred.


* Demonstrates C.A.R.E.
