MDS Nurse (RN/LPN)

We are seeking a dedicated and detail-oriented MDS Nurse (RN or LPN) to oversee the assessment and care planning process for our residents.
Responsibilities:
• Coordinate and complete the MDS (Minimum Data Set) assessments in accordance with state and federal regulations.
• Ensure accuracy and timeliness of documentation for resident care plans.
• Collaborate with interdisciplinary team members to develop individualized care plans that promote resident well-being.
• Monitor changes in resident status and update MDS assessments as required.
• Participate in quality improvement initiatives and compliance audits.
Qualifications:
• Current RN or LPN license in the State of Ohio.
• Prior experience in MDS coordination or long-term care preferred.
• Strong organizational and communication skills.
• Knowledge of Medicare/Medicaid regulations and reimbursement systems is a plus.
