Social Service Assistant

San Bruno Skilled Nursing is a 50 bed Post-Acute facility, we believe in a patient-centered approach and are looking for Social Services Assistant to join our team.

Full Time

3 days Weekday, Saturday and Sunday

$25/Hr

Responsibilities:


* Collaborate with residents and their families to establish goals and develop a course of action that promotes self-care, independence, and overall well-being.


* Provide medically related social services to support the highest practicable physical, mental, and psychosocial well-being of each resident.


* Assess social, family, psychological, and emotional needs to develop comprehensive care plans addressing social services issues.


* Offer emotional support and guidance to residents and families facing challenges such as coping with physical disabilities, health-related fears, and grief associated with loss or death.


* Actively participate in discharge planning and coordinate with appropriate community agencies and referrals as needed.


* Interview residents, guardians, and sponsors to obtain required admission information and signatures


* Prepare identification records for residents according to established policies and procedures


* Maintain an accurate record of available beds


* Admit, transfer, and discharge residents in adherence to established policies and procedures


* Provide residents with admission information packets, including resident rights, notice of privacy practices, and admissions contract

Requirements:


* High school diploma or equivalent required.


* Experience in skilled nursing and familiarity with PointClickCare (PCC) preferred, but not required.


* Demonstrated ability to work effectively in a fast-paced environment and prioritize tasks efficiently.


* Excellent communication and interpersonal skills, fostering positive relationships with residents, families, and staff.


* Ability to evaluate social and family information, psychological, and emotional needs to assist in assessing social services needs as well as develop care plans for social services issues.
