? What You'll Do


* Deliver personalized physical therapy services to post-acute and long-term care residents


* Collaborate with a multidisciplinary team to develop and implement care plans


* Help patients improve mobility, strength, and overall physical function


* Document progress and maintain compliance with regulatory standards


* Educate patients and families on therapy goals and home exercises
