Clinical Assessment Coordinator (Hybrid - Northern Illinois)

Maximus is hiring a Clinical Assessment Coordinator to support our Outreach & Assessment team! This role will conduct in-person interviews and evaluations to determine the need for referrals to alternative services for individuals with special and complex needs.

They will identify what the person may require to live in the community, assessing their needs, strengths, abilities, preferences, and whether they can safely and successfully transition to community living.

About the program: The Outreach & Assessments (O&A) Program at Maximus supports individuals seeking long-term services and supports by providing person-centered outreach, completing comprehensive assessments, and guiding applicants through eligibility and care options.

The program works closely with families, providers, community organizations, and state agencies to ensure individuals understand available services, meet required criteria, and receive timely, accurate information throughout the process.

Through strong customer service, adherence to program requirements, and effective coordination, O&A staff play a vital role in improving access to care and supporting Maximus' mission of helping government serve the people.

This position requires up to 80% travel within Northern Cook/Lake County IL and the position does not require a nursing license and has no nursing-related responsibilities.
